Individual care plan

Short definition

An individual care plan is the document in which the team records, for each service user, the assessed needs, goals, interventions to be carried out, who is responsible and review dates.

In plain language

In Portugal it is a standard tool in social care services for older people, such as residential homes, day centres and home care; in the continuing care network it is called an individual intervention plan. It should be drawn up with the participation of the person and their family and reviewed when the situation changes. Activities such as reminiscence or cognitive stimulation may be included in it.
