Short definition

GRADE is an international system for rating the certainty of evidence on four levels (high, moderate, low and very low) and the strength of clinical recommendations.

In plain language

In GRADE, evidence from randomised trials starts at high certainty and evidence from observational studies at low certainty. It is then rated down for risk of bias, inconsistency between studies, indirectness, imprecision or publication bias, and can be rated up, for example, for a very large effect. It is used by Cochrane, the WHO and many organisations that produce clinical guidelines.
